Flu Shot $5 Rebate Form

If you are planning on getting a flu shot in the next few weeks:

Lysol is offering a $5 Flu Shot Rebate

Simply mail in the completed the rebate form found here along with the original dated receipt showing payment for the flu shot. Keep in mind that this rebate is ONLY valid for flu shots received from 11/7-11/30 and must be postmarked by 12/30/10. Limit of one offer submission per name, address or household; valid everywhere in the United States except Rhode Island and no purchase of Lysol products is required!

Keep in mind that some stores will reward you with a coupon booklet (i.e. Rite Aid) or other perks for getting the flu shot, so be sure that you ask the pharmacist for more details.
